As a healthcare service provider we need to be able to take certain details from a patient that include such things as:
• Name and address
• Telephone numbers and e-mail address(es)
• Medical history and medication details and observation notes after an appointment
• Next of kin contact details if deemed appropriate by your practitioner - for example, if the patient is under the age of sixteen. It is your responsibility to ask permission of any third party before you give us their personal data.
All information will be kept to the essential minimum and only taken as part of an ongoing care plan for the podiatric issue you have.
Your personal treatment and observation data is stored as physical records. Your contact details are stored on the physical record and also electronically on a secure computer system.
Physical records are kept within a secure locked location, and are used only the purpose(s) for which it has been collected. The only person who has access to your records is the person treating you. Canon Barn Podiatry will collect and store the essential data needed in order to treat you. It will be kept up to date and we will only keep it as long as it is necessary by law after you have ceased treatment.
Your records are confidential. Occasionally it may be necessary to share information with other healthcare professionals such as other podiatrists or your GP. This will only be done with your knowledge and consent and when it is in your best interests in order to provide you with the most appropriate level of care.
